Direct Care Wellness Counselor I, II, or III provides outreach supported community living assistance to individuals living in an independent setting (house, apartment, etc.) Our staff will assist each client member with their individual life vision; while developing wellness & stability and living independently in their community.
Our Direct Care Wellness Counselor Staff:
Most all of our positions require working with individuals that are diagnosed with chronic mental health conditions (for example: anxiety, depression, bi-polar, schizophrenia, some multiple diagnosis, etc.). These positions require meeting our client members in their homes or community providing them with support and personal development skills, empowering them toward their life vision and assisting them with management of stabilizing their mental health conditions with loving kindness, and care. These roles combine direct care and administrative duties (documentation, service plans, team meetings, etc.) as a component to the role and care process.
Our Direct Care Wellness Counselor Staff:
- Builds trusting-caring relationships and healthy rapport with client members for a stable counselor-client member supportive relationship.
- Support client members with maintaining mental health stability
- Assist client members with cultivating personal development skills (for example; coping skills, transforming negative patterns and behaviors, self management skills)
- Help client members with identifying and removing barriers to managing mental health symptoms through solution based outcomes
- Fosters the development of natural supports and social-integration; while working with the client member side by side
- Support client members with maintaining focus and attention to personal life goals; as client members have chosen for themselves
- Works with client members on sustaining a healthy physical, mental, emotional, and purposeful lifestyle.
- Helps client members to access and utilize community resources and natural support systems
- Assists client members with sustainable independent living skills for grounding a stable and functional lifestyle (financial management, organization and clean living environments, daily/weekly schedules, maintaining appointments)
- Provides client members with encouragement and direct support in exploring and pursuing their dreams
